Clinical Case Manager
Reports to Director of Public Health Initiatives, this position is responsible for monitoring and follow–up of all chronic care patients, including those for diabetes patients, cancer screenings, patients without clinical visits in past six months, patients diagnosed with depression, asthma and obesity diagnosis etc. The case manager is also responsible for providing documentation in the patient record, assisting in planning and implementing programs and projects, and preparing reports and evaluations in addition to any other services that is needed.
Education/Training
- A minimum of an Associate’s degree in social work, nursing or public health or five years of related experience.
- Bilingual capability strongly preferred but not required.
