Public Housing Primary Care
WSHC in collaboration with the BHA and a consortium of other partners have developed a program to improve the health and well being of residents in public housing and the larger community. Social Health Care Coordinators work with on site Tenant Tasks forces to provide screenings, referrals and primary care to address social and environmental factors contributing chronic diseases such as diabetes, hypertension and depression.
