National Diabetes Awareness Month

  • November 7, 2025

November is National Diabetes Awareness Month, a time to raise awareness about diabetes and its impact on individuals and communities. In the United States, over 38 million people are living with diabetes, yet nearly one in five remain undiagnosed1. While traditional risk factors like age, family history, genetics, and physical inactivity are well-known, emerging research highlights the significant role of Social Determinants of Health (SDOH) in influencing diabetes risk2. Key SDOH influencing diabetes risk include:

  • Income and Housing Stability: Limited financial resources can restrict access to healthy foods and healthcare services, increasing diabetes risk.
  • Access to Healthy Foods: Living in “food deserts” with limited access to affordable, nutritious food options can lead to poor dietary habits contributing to obesity and diabetes.
  • Education and Health Literacy: Lack of education and understanding about healthy lifestyles can hinder effective diabetes prevention and management.

At Whittier Street Health Center, we recognize the profound impact of social determinants of health on diabetes risk and are committed to addressing these factors through comprehensive, patient-centered care. Our Adult Medicine Department takes a multifaceted approach to diabetes management, offering regular screenings to identify individuals at risk, personalized nutrition counseling to guide healthy eating habits, and medication support to help patients maintain optimal blood sugar levels. In addition, we provide education and group visits that empower patients and their families with the knowledge and tools needed to prevent and manage diabetes effectively. To further support patients, our Remote Patient Monitoring (RPM) Program helps individuals with uncontrolled diabetes or hypertension track their vital signs from home, providing real-time insights and personalized guidance for better health outcomes.

This American Diabetes Month, take the first step toward managing your health: learn about your risk, schedule a screening, and connect with our providers for the support and tools you need to live to your fullest potential. To register for a group session, please call (617)-858-2433. For more information about the RPM program, please call (617)-989-3065.
